You can get the ESIC Form-9 to make a Claim in case of Maternity Benefit as provided by the ESIC, So, through Company HR you will be guided by esic representative appointed by company.

Dear sir,
Today dt 12.01.2023 i&my wife visited Kandiwali East ESIC hospital at 12.08PM to take appointment of Ortho doctor. But they refuse to give appointment by telling that they will give appointment at 8am to 12pm. I don’t understand why giving appointment for only 4 hours. If Doctor available full day,then what is the issues. I request the security guard that we stuck in traffic but they denied.
My request to Govt athority to look into the matter.

प्रिय सर कदी वाली हॉस्पिटल पहली बार गया था जो कि वहां के स्टाफ का व्यवहार बहुत ही खराब है मेरी औरत प्रेग्नेंट है मैं उसको इलाज के लिए ले गया था एक डॉक्टर के कहने पर कुछ टेस्ट करने को था लेकिन 6 दिन हो गए अभी भी लोग दौरा रहे हैं आ जाना कल आना मैं क्या करूं समझ में नहीं आ रहा है पैसे लेते तो पैसे दे देता मैं मैं वसई से कांदिवली रोज आता हूं यह नाम के लिए कामगार हॉस्पिटल है इससे अच्छा तो सरकारी हॉस्पिटल होता है मैं पहली बार किसी काम का हॉस्पिटल में गया था लेकिन मेरे मन को बहुत ठेस पहुंचा यह सिर्फ नाम का कार्य कामगार हॉस्पिटल है यहां काम तोरा सरकारों के सरकारी अस्पताल से भी बुरा है इससे अच्छा तो सरकारी अस्पताल होते हैं कम से कम एक-दो दिन ही जाना पड़ता है जांच तुरंत हो जाती है थोड़ा पैसा लगता है जितना पैसा मेरा इस आईडी में कट चुका है उतना पैसा अगर मैं अपनी जेब में रखता तो आज मुझे यह दत्ता खाने की नौबत नहीं आती
कांदिवली हॉस्पिटल के स्टाफ का व्यवहार बहुत ही खराब है मैं भी एक दो बार वहां इलाज के लिए गया था लेकिन जिस से भी मिला जो लोग वहां इलाज कराने आए हैं सब यही बोले कि बाहर करा लो यहां परेशान कर देंगे रोज दौड़ दौड़ पर आओगे अच्छी है लेकिन लोगों के इलाज में काम नहीं आ रही स्टाफ चाहते हैं कोई यहां आए ना हम बैठकर पगार ले इसकी वजह से उन लोग सब को परेशान करते हैं ताकि कोई यहां आए ना हॉस्पिटल की सुविधा बहुत अच्छी है बनावट बहुत अच्छा है लेकिन गरीब मजदूरों के किस काम का जो उनको यहां इलाज ही ना मिले पैसा काटने के लिए काट लेते हैं मैं तो सभी लोग से यही कहूंगा कि वहां मत जाओ लालच में कि सब यहां फ्री है ना यहां जाने का मतलब समय की बर्बादी और कुछ नहीं अच्छा आप लोग किसी राज्य सरकार के सरकारी हॉस्पिटल में इलाज करवा लेना सिर्फ यह हॉस्पिटल दुनिया को दिखाने के लिए है कि देखो हम मजदूरों को कितना अच्छा इलाज दे रहे हैं लेकिन हकीकत में सिर्फ यहां मजदूरों को परेशान किया जाता है और कुछ नहीं
